Attendance*

I understand that I am responsible for ensuring my child’s attendance at the program.
I understand that my child is to arrive on time and attend Tuesday rehearsals regularly.
They can attend the mandatory concert on Saturday, May 2, 2026 at 4 pm.

Behavioral Agreement*

I understand that I am responsible for my child’s behavior and conduct during program activities. I understand that my child is expected to behave in a way that is respectful to the program’s staff, participants, rehearsal space, and materials. I understand that, should my child not adhere to the code of conduct, disciplinary action such as a conversation with the student, conversation with the parent(s), temporary suspension, or removal from the program is possible.

Uniform Care*

I understand that a uniform will be provided, and that I am responsible for caring for the uniform until it is returned at the end of the season.

Tuition*

I understand that tuition is non-refundable.

Parental Consent Agreement and Acknowledgment of Risk*

I, the undersigned, acknowledge that I have voluntarily enrolled my child in the Delaware County Children’s Chorus, which may involve physical activities or other risks. I understand that there are inherent dangers associated with such activities, including the possibility of injury, illness, or even death, despite precautions taken.
I hereby consent to my child’s participation in this activity, and I assume full responsibility for any risks, injuries, or damages that may result from my child’s participation, whether caused by accident, negligence, or otherwise. I understand that the organizers, instructors, and facility owners will take reasonable measures to ensure safety, but I accept that there are no guarantees of absolute safety.
I waive any claims against the organizers, their employees, agents, and affiliates for any injury, death, or loss suffered by my child while participating in the activity. I further agree to follow all safety instructions and guidelines provided by the organizers and to ensure that my child is in proper physical condition to participate.
